Just the lens of the laser and the light shining on the eye. Your eye lids are held open and you look straight ahead. I've had it done in both eyes. You do feel a bit of a sting in the eye and if it is a rapid succession then the burning feeling is a bit more intense. It really isn't all that bad to go through and is well worth it if you can save your eyesight.
